The Mixing Log Template is intended solely for use by the medical practice or practitioner to whom it is addressed. Nationwide Allergy Arizona, LLC provides consulting services and facilitates access to a sterile compounding pharmacy that supplies allergens for allergy immunotherapy to be mixed by licensed practitioners. The provided Mixing Log Template is a reference document designed to assist in meeting documentation requirements for allergy immunotherapy preparation and administration. Once completed by your office, it constitutes your official Mixing Log, and it is your sole responsibility to accurately complete, verify, and maintain this document. While Nationwide Allergy Arizona, LLC strives to ensure accuracy in information regarding source allergens (including their names, producers, lot numbers, and expiration dates), it remains your office’s responsibility to verify all information before submission to third parties, including insurance payors. Practices must properly document: Number of units mixed, Dates of service, Dates of mixing. Nationwide Allergy recommends regular internal audits to ensure accuracy and compliance. Nationwide Allergy Arizona, LLC disclaims all liability for errors, omissions, or inaccuracies in the completed Mixing Log or for any claims, billing submissions, or reimbursement denials related to its use. The use of this document is voluntary and at the discretion of your office. It is your office’s responsibility to ensure that all documentation complies with federal, state, and local laws, as well as pay or specific requirements. Nationwide Allergy Arizona, LLC does not oversee, supervise, or assume responsibility for the actions of third parties, including: Sterile compounding pharmacies, Insurance payors and others. Any errors arising from third-party actions or documentation must be addressed directly with those entities. Nationwide Allergy Arizona, LLC serves strictly as a consulting service and a conduit to facilitate access to a sterile compounding pharmacy. We do not directly provide or prepare allergens and do not dictate how your office prepares or administers allergy immunotherapy. Use of the Mixing Log Template or any guidance from Nationwide Allergy does not establish a legally binding relationship beyond consulting services.
